I understand that parts of the MBRC Challenge/Adventure Course may be physically and/or emotionally demanding. I affirm that l/my child is in good health and not under a physician's care for any undisclosed condition that bears upon their fitness to participate in this activity.

I understand that the level or participation in this activity is at all times completely voluntary and subject to the participant's choice. In addition, I recognize the inherent risk of injury or disability in this activity and understand that each participant must assume the risk of injury that could result from any of the activities. I release the Marshall Baptist Retreat Center, the Marshall Baptist Association, and their staff members, contract staff, principals, employees, and volunteers from all liability for any injury to me/my child during this activity. Also, in the event of an emergency, I hereby give my permission to the MBRC staff (or emergency response personnel) to select a medical facility, physician, and/or otherwise secure proper treatment for me or my child should I be unable to communicate my specific desires. (If I have exceptions to such treatment/hospitalization, I understand that I may note them here and agree to hold to only myself liable for these noted exceptions.)
